Winfield Receives $500 Grant to Support Fire Safety Open House

Dear Grant Award Recipient,

On behalf of the Maryland State Firemen's Association and the Office of the State Fire Marshal, we would like to inform you that your department’s application for the 2014 MSFA Fire Prevention Grant Program has been approved.

Based upon your submission, the MSFA will be providing support to your project in the amount of $500.00 with your company match requirement of 20% plus the remaining balance over the $600.00 invoice amount. It is our understanding that your department will be purchasing various fire prevention paraphernalia with these funds. Companies selecting to use multiple vendors will receive checks for vendors to equal grant funds being provided.
The MSFA check(s) will be cut by the treasurer and made payable directly to your identified vendor(s) for the materials being acquired for this fire prevention activity. You will receive your check at the MSFA Convention.

**REMINDER** Checks provided from the MSFA will be provided ‘one time only’ to the selected departments. If the check provided is misplaced and/or lost, a new check will not be re-issued. It is the sole responsibility of the department representatives to maintain control of funds provided.
If there are any changes to the previously submitted invoice (item no longer available, price change, etc.), you must send those documents directly to MSFA Treasurer, Ron Siarnicki at 2130 Priest Bridge Drive, Suite 6, Crofton, MD 21114 or rsiarnicki@aol.com or by fax to 443-249-0456 in order to facilitate this process. Proposed purchase order changes must be received by June 6, 2014. These purchase orders must be copies of the form you will be submitting to the vendor for items to be purchased.
